Job Summary:
Under the general supervision of the Center Head and with some latitude for the exercise of independent judgement, performs somewhat difficult, responsible professional work requiring training, some experience and broad knowledge on case management, group sessions and counseling; and does related tasks that maybe assigned from time to time.

Primary Tasks / Terms of Reference:
- Interviews children and accomplishes Intake Sheets;
- Manages individual cases of children by doing the following:
- Prepares Rehabilitation Report.
- Conducts visit/monitor to get more information from the children’s family and significant others;
- Prepares initial/updated social case reports;
- Prepares other documents;
- Presents cases during matching conference;
- Prepares progress notes/recordings of cases;
- Places children in foster care/adoption/reintegration to their families and relatives;
- Accompanies children for placement in foster care/ICAB and for medical purposes;
- Prepares transfer summary/closing summary report of cases
- Conducts group sessions to RSCC children.
- Conducts counselling sessions to RSCC children.
- Attends/conducts admission/discharged and cases conference.
- Attends meeting, seminars conducted by the DSWD Central Office/Field Office/ICAB, and;
- Performs other related tasks as maybe assigned by the supervisor.
