Job Summary:
Under the general supervision of the Center Head and with some latitude for the exercise of independent judgement, performs somewhat difficult, responsible professional work requiring training, some experience and broad knowledge on case management of clients involving treatment plan development, implementation and evaluation and coordination with partners; and does related tasks that maybe assigned from time to time.

Primary Tasks / Terms of Reference:
- Assists the Center Head in the overall operation of the residential care facility in the areas of Organization and Administration, Program Implementation, facility maintenance, as needed;
- Handles case management of clients involving treatment plan development, implementation and evaluation;
- Implements rehabilitative services based on the treatment plan and prepares corresponding minutes of the plan implementation to Center Head;
- Maintains files, social case study reports and databank of clients served;
- Prepares social case study report and conducts home visit and extend assistance and/or services to families through the self-help concept;
- Attends and assists clients during court hearings when necessary;
- Establishes and updates data banking of these related cases as reference form policy enhancement and other program implementation;
- Monitors and follow-up case management of LGUs as to services provided to discharged clients/ensure after-care service are provided;
- Performs other related tasks that may be assigned from time to time by Center Head/Regional Center Coordinator.
