Job Summary:
The Social Welfare Assistant (SWA) coordinates with the health and school facilities and is responsible for distribution and retrieval of Compliance Verification (CV) Forms of the Pantawid beneficiaries. The Social Welfare Assistant also assists the Municipal Link in her/his workload and management of cases e.g. referrals and coordination in the barangays.
Job Outputs:
- Summary report of distributed and retrieved Compliance Verification (CV) Forms;
- Summary report of collated Grievance Redress Forms;
- Summary report of monitored facilities;
- Summary report on the issues and concerns raised by the facilities visited.
Primary Tasks/Terms of Reference:
1. Facilitates distribution and retrieval of Compliance Verification forms;
2. Prepares summary of Compliance Verification Forms distributed and retrieved, and taken note of issues and concerns on meeting the CVS distribution and retrieval and submission to C/ML for appropriate action;
3. Facilitates coordination with schools and health centers relative to CVS monitoring and retrieval;
4. Collates submitted updates forms in terms of complete attachment of documents/requirements;
5. Collates submitted GRS forms;
6. Assists the Municipal Link in her/his workload and management of cases e.g. referrals and coordination in the barangays;
7. Performs other related tasks as may be assigned.
